Admitted Students

Once accepted into UNDERC Education Programs, the following need to be submitted prior to participating in the course:

Deposit, remitted to Sherry DePoy at the Campus address
I-9 form, filled out online via InsideND and completed by an authorized representative
W-4 federal tax form, filled out online via InsideND
W-H Indiana tax form, filled out online via InsideND
Direct deposit form, filled out online via InsideND
Chemical and Laboratory Safety Training, completed online via ComplyND
Occupational Health Program Category A form, through Notre Dame
Institutional Animal Care and Use Committee (IACUC) Training, through Notre Dame
- Working with the IACUC
- Introduction to Amphibians
- Introduction to Wildlife
